In regards to the '3 penalties in a race weekend', I'm pretty sure Badoer picked up three speeding penalties in Valencia whilst subbing at Ferrari.

Also, Norberto Fontana picked up 3 speeding penalties in his first race for Sauber in 1997.

Probably a few more out there...
